According to the Naruto manga, chapter 92, page 18, the given name before the place where Naruto inscribes his own is translated to either 'Arashi', 'Kaseiyo', or 'Kayaku'. Many fans insist that one of these names is indeed the Fourth's, but nothing can be proven for sure.
Other fans speculate that Yondaime's clan name/last name (if indeed he comes from a clan) is Kazama, which, in retrospect, would have more of a meaning. By leaving Naruto as an orphan with the last name of the famous Fourth Hokage, he would have been made an even bigger target. After all, the Fourth Hokage was known for annihilating hundreds of Stone-nins in the most recent 'Great Shinobi War'. If Naruto had kept the same last name as him, many of the Fourth's enemies would seek revenge on the blonde haired young ninja, regardless of whether or not Naruto actually is the Fourth's son.
